Since the King's illness-related collapse 10 days ago, No one else is allowed to see the King, according to Queen Jo (Kim Hye-Jun) and her father Chief State Councilor Jo Hak-Joo (Ryoo Seung-Ryong). Even seeing his own son, Crown Prince Lee Chang (Ju Ji-Hoon), is prohibited. Joseon is being ruled by Chief State Councilor Jo Hak-Joo with an iron fist, yet there are whispers that the King has passed away.
As he slips into the King's palace, Crown Prince Lee Chang detects something evil. He senses the vile stench of a shadowy beast he sees through a door, but he is found and led away from the palace. For the King, his bodyguard Moo-Young (Kim Sang-Ho) snatches the medical magazine. The journal is almost finished.The journal is almost completely empty, although it has the name of Doctor Lee Seung-Hui (Kwon Bum-Taek). Crown Prince Lee Chang and Moo-Young visit the doctor at the Jiyulheon clinic in remote Dongnae to learn what happened to the King. When they get there, they see that the clinic is destroyed and that there are dead bodies under a structure. Seo-Bi, a member of the medical staff, is the only person to have survived the clinic, as Crown Prince Lee Chang and Moo-Young subsequently discover (Bae Doo-Na). When they do locate Seo-Bi, she informs them that the people they believed to be dead at the clinic are actually alive and will soon rise to commit murder.
