Two attractive but naughty American girls are travelling through Europe by car. They find themselves in the woods alone at night in Germany with a broken car. They locate a remote villa while looking for assistance. When they wake up the following day, they and a Japanese man are imprisoned in a spooky makeshift basement hospital. The older man from Germany introduces himself as a retired surgeon with experience splitting up Siamese twins. But instead of being split up, his three "patients" are going to be combined in a horrifying procedure. His goal is to establish the first connection between people through their digestive systems. He intends to do this in order to realise his deranged lifelong fantasy of the human centipede.
